If you wish to use the facilities at Faith Assembly please download and fill out the Facilities Request Form and return to John Morabito, Director of Facilities for approval.

Vehicle Request Form

If you wish to use a Faith Assembly vehicle please download and fill out the Vehicle Request Form and return to Joyce Jones for approval.

Forms may be dropped off during church office hours (9-5, M-F) or emailed by clicking on the underlined names above.
